Your ApoB came back at 118. Let's put that in context, because it matters more than the LDL number you were probably looking at.
My doctor said my cholesterol was fine though?
Your LDL-C is 132 — borderline, and by most Indian lab reference ranges it gets flagged as 'near optimal'. But LDL-C measures the cholesterol mass inside your LDL particles. ApoB counts the particles themselves, and there is exactly one ApoB per atherogenic particle.
That distinction matters because atherosclerosis is a particle-number disease. A particle has to cross the endothelium and get retained for plaque to form — the cholesterol it happens to be carrying is secondary. In people with insulin resistance, and yours is real at HOMA-IR 3.1, particles run small and dense: more of them for the same cholesterol mass. So LDL-C understates your risk.
The second thing: risk is cumulative exposure. Not today's number — the area under the curve across decades. You are 38, and you have gone 96 → 104 → 111 → 118 since 2023. Held flat, that's roughly 2,700 mg-years of exposure by 60. Down at 70, it's about 1,700.
